How to read your prescription?

Your prescription must be less than 2 years old or 1 year if you are 19 years old or under or 65 years old and up. the prescription must contain: Your name. The Doctors name. The date of the eye exam. OD (right eye measurement). OS (left eye measurement). ADD for near vision (if you are 40+ years old) PD measurement (if provided)
